High School Hero Nomination Form

Help us recognize outstanding high schools students in your area by nominating yourself or someone who you believe is deserving of our High School Hero Award.
Nomination Criteria
Good academic standing
Must be a freshman, sophomore, junior or senior in high school
Highly involved in both community and school
Person goes "above and beyond" to promote school spirit, serve the community, help others, and is a positive role model to others
Nomination Process
Below is the High School Hero nomination form. Please complete the form and its entirety. Forms may either be submitted online or printed and mailed to Franklin Building Supply’s Corporate Office at 9222 Barnes Dr. Boise, ID 83709 Attn: High School Hero Committee. Selection Process
Each season, the HSH Committee will meet to review the nominations. Nominees will be judged on the basis of multiple achievements:
Grade point average (25%)
Academic activities (25%)
School related extra-curricular activities (25%)
Community and/or civic related involvement (25%)
Award
Franklin Building Supply will donate $500 in student's name to the academic department or non-profit 501(c)(3) organization of his or her choosing. The winner will also receive a $500 cash prize for themselves, along with press and media coverage provided by Franklin Building Supply.
